Why are date of birth and address required?

Insurance pricing and eligibility can depend on age and where a vehicle is kept. These details also help the licensed recipient match the request to the correct person and rating territory.

Could the provider obtain insurance-related reports?

Not from this intake form alone. Because the licensed recipient is assigned after submission, this form does not authorize an unnamed party to obtain private motor vehicle, claims, prior-insurance, credit-based insurance, or other consumer reports. The assigned recipient may send a separate disclosure or authorization identifying itself when one is required.

Does submitting bind insurance coverage?

No. Coverage begins only after a licensed provider or insurer confirms the terms, completes any required verification, and issues or binds a policy.

Where is the service currently available?
